Angioimmunoblastic T-cell lymphoma, or AITL, presents a difficult diagnosis to handle. This rare blood cancer affects T cells, a type of white blood cell responsible for fighting infection. When these cells grow abnormally and multiply out of control, they often build up in the lymph nodes and spread to other parts of the body. AITL is a subtype of non-Hodgkin lymphoma, and while it can occur at any age, it most commonly impacts older adults. Stage III indicates involvement of lymph nodes on both sides of the diaphragm, while stage IV means the cancer has spread to organs such as the liver, bone marrow, or skin. Most people are diagnosed at one of these later stages.

Because AITL grows quickly, treatment usually starts soon after diagnosis. For early-stage disease, combination chemotherapy is standard, often paired with radiation to the affected areas. Advanced-stage disease is typically treated with the CHOP regimen, a combination of Cytoxan (cyclophosphamide), doxorubicin (hydroxydaunorubicin), Oncovin (vincristine sulfate), and prednisone. Another option, BV+CHP, incorporates Adcetris (brentuximab vedotin). Steroids may be added early to help manage immune-related symptoms like joint pain or rash.

For those whose disease responds to chemotherapy, a stem cell transplant might follow to lower the risk of recurrence. This procedure can use the patient’s own stem cells or those from a donor. Targeted therapies addressing specific genetic changes are also used, especially in relapsed cases. Given the rarity of the condition and the constant development of new treatments, clinical trial participation is frequently encouraged. Regular follow-up appointments, imaging, and blood work remain essential long after treatment ends.
