
Hospitalists are shifting their approach to treating unhealthy alcohol use, focusing on patient-centered care and better risk assessment. The latest strategies for treatment of alcohol withdrawal emphasize removing stigmatizing language from medical records and using validated tools to predict withdrawal severity. Presenters at the session argued that these changes can improve engagement and align with modern clinical standards.
Alcohol withdrawal remains a frequent inpatient issue. About half of patients with unhealthy alcohol use experience some form of withdrawal, though 80% of these cases are uncomplicated. Severe complications, including seizures, hallucinations, and delirium tremens, affect 5% to 20% of patients. The session highlighted the concept of “kindling,” a pathophysiologic mechanism where repeated withdrawal episodes increase the risk of severe reactions in the future.

To manage this variability, clinicians are turning to the Prediction of Alcohol Withdrawal Severity Scale, or PAWSS. A score of 4 or higher on this tool demonstrates excellent sensitivity and specificity for predicting severe withdrawal, allowing hospitalists to triage patients appropriately. While the Clinical Institute Withdrawal Assessment for Alcohol (CIWA) has historically been the standard, it is subjective and requires frequent nursing assessments. Presenters noted that more than half of hospitalized patients are evaluated with this protocol unnecessarily. The modified Minnesota Detoxification Scale (mMINDS) was presented as a more objective alternative, validated across emergency and intensive care settings, though clinicians must interpret scores carefully because unrelated vital sign abnormalities can inflate results.
Phenobarbital is increasingly used as an adjunct for severe or refractory cases. Its dual mechanism—enhancing gamma-aminobutyric acid activity and inhibiting N-methyl-D-aspartate receptors—makes it effective where benzodiazepines alone may fail. Evidence suggests it helps reduce intensive care unit admissions and shortens hospital stays. However, its narrow therapeutic window and toxicity risk require careful dosing and monitoring. Combination therapy with benzodiazepines may further improve outcomes in these difficult scenarios.
Medication for Alcohol Use Disorder
Despite the strong evidence that medication for alcohol use disorder (MAUD) reduces mortality, hospital readmissions, and alcohol consumption, fewer than 1% of eligible patients are discharged on these therapies. Hospitalization is framed as a “reachable moment” to initiate treatment. Naltrexone is highlighted as the first-line agent with the strongest evidence base. It reduces cravings and drinking behavior and can be safely used in patients with mild to moderate liver disease, including Child-Pugh classes A and B. It should be initiated once a patient is no longer acutely withdrawing and has abstained from alcohol for at least 48 hours. Starting at a lower dose helps mitigate side effects like nausea and insomnia.

Acamprosate is another FDA-approved option for maintaining abstinence, though its three-times-daily dosing and cost can limit adherence. Disulfiram relies on aversive conditioning and does not treat underlying cravings, making it less effective for many patients. Gabapentin is commonly used off-label as an adjunct for withdrawal symptoms and protracted withdrawal, while topiramate has evidence for reducing heavy drinking days but carries cognitive side effects. Emerging therapies such as glucagon-like peptide-1 receptor agonists are under investigation.
Adjunctive therapies also play an important role. All patients should receive IV thiamine to prevent Wernicke’s encephalopathy. Clinicians should routinely assess magnesium levels, liver function tests, and urine drug screens. Vigilance for coexisting conditions, including psychiatric disorders and high suicide risk, is also essential. The session emphasized that hospitalists play a critical role not only in managing acute withdrawal but also in initiating long-term treatment for alcohol use disorder. By using validated risk stratification tools, optimizing withdrawal management strategies, and consistently offering MAUD, clinicians can significantly improve patient outcomes.
